I was once on a works committee.  There had been a number of accidents involving the double set of doors to the ladies loos.

This came out in the minutes as
Mrs Jones from Packing asked for a window in the door of the ladies toilets.  The Management said they would look into it.

Does anyone else have any amusing minutes?
